Job Description

Life Care Centers is seeking a compassionate, patient-focused Certified Occupational Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will work under the supervision of a licensed Occupational Therapist to provide hands-on rehabilitative care for primarily senior patients in a post-acute, skilled nursing, and long-term care environment. You will carry out individualized treatment plans designed to improve functional independence, safety, and quality of life, with a strong focus on activities of daily living such as dressing, grooming, bathing, and functional mobility. As a Certified Occupational Therapist Assistant with Life Care Centers, you will assist with patient evaluations by gathering information, observing functional abilities, and reporting findings to the supervising therapist. You will implement therapeutic exercises and activities, guide patients in the safe use of adaptive equipment, and reinforce techniques that support strength, coordination, balance, and cognitive engagement. You will carefully monitor patient responses, document progress in the electronic medical record, and communicate changes in condition or performance to the therapy and nursing teams. You will collaborate daily with Occupational Therapists, Physical Therapists, Speech-Language Pathologists, nurses, social workers, and other members of the interdisciplinary team to support coordinated, patient-first care. Your work will include organizing and managing a treatment schedule, preparing therapy spaces and equipment, maintaining infection control and safety standards, and educating patients and families on home exercise programs and strategies for maintaining independence after discharge. Life Care Centers of America specializes in senior living and rehabilitation, and this position reflects our commitment to compassionate, high-quality care in a family-focused environment. You will contribute to a culture that prioritizes respect, dignity, and meaningful engagement for every resident. We offer a team-based environment that supports professional growth, mentorship from experienced clinicians, and opportunities to participate in quality-improvement initiatives aimed at achieving excellent clinical outcomes. The ideal candidate is a Licensed/Certified Occupational Therapist Assistant who is comfortable working in a fast-paced, medically complex setting, is attentive to detail in documentation, and brings empathy, patience, and clear communication skills to daily interactions with residents and their families. You should be able to safely assist with transfers and mobility tasks, follow established treatment protocols, and adapt activities to each patient’s unique abilities, preferences, and goals. This is an on-site clinical role that requires consistent presence in the facility to deliver direct patient care. In addition to competitive pay, Life Care Centers typically offers a comprehensive benefits package that may include medical, dental, and vision coverage, life insurance, retirement savings options such as a 401(k), paid time off, and access to ongoing training and professional development. If you are a dedicated Certified Occupational Therapist Assistant looking to make a meaningful difference in the lives of seniors while advancing your career in a supportive, collaborative environment, we encourage you to apply.
