LSF is seeking a talented SNAP Case Manager who wants to make an impact in the lives of others. This Case Manager will be working directly with youth ages 12-17 who are dually involved or at risk of being dually involved in the DCF and DJJ Systems. The Case Manager will run groups for youth ages 12-17.
Purpose and Impact:
Responsible for the day-to-day case management and system navigation of SNAP®.
Staff in the dually served program will be working with youth ages 12-17 who are dually involved or at risk of being dually involved in the DCF and DJJ systems. This case manager will be primarily working directly with youth and running groups for youth ages 12-17. This role additionally requires that someone be tech savvy as you will guide youth through the SNAP program using a tablet.

A Bachelor’s degree in Social Work or a related field.
Experience:
1-3 years’ experience with structured groups and parent education groups is required; community/agency liaison experience is desired.
