Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS) nurse to support our Healthcare & Medical Services department within our senior care and post‑acute settings. In this role, you will be responsible for coordinating and completing comprehensive MDS assessments to ensure accurate clinical documentation, regulatory compliance, and appropriate reimbursement for residents across our skilled nursing and senior living facilities. Working closely with interdisciplinary team members—including physicians, advanced practice providers, therapists, social workers, and bedside nursing staff—you will gather, analyze, and validate clinical data to reflect each resident’s current condition, functional status, and care needs. You will enter and review MDS data within electronic health record systems, verify coding accuracy, and monitor timely submission of assessments in accordance with federal and state regulations, including CMS guidelines. The RN, MDS will assist in developing, updating, and evaluating individualized care plans based on MDS findings, quality measures, and resident goals, helping to improve outcomes, reduce rehospitalizations, and support quality initiatives across the continuum of care. You will serve as a clinical resource for MDS processes, providing education and guidance to staff on documentation practices, RAI processes, quality indicators, and policy updates. This position requires exceptional attention to detail, strong critical thinking skills, and the ability to interpret complex clinical information and regulatory requirements. Successful candidates will be comfortable working with data, collaborating across disciplines, and prioritizing multiple deadlines in a fast‑paced environment.
