Sanford Health is seeking a dedicated RN, Minimum Data Set (MDS) nurse to support high-quality resident care and regulatory compliance within our Healthcare & Medical Services department. In this role, you will use your clinical expertise and assessment skills to coordinate and complete comprehensive MDS assessments for residents in our senior living and post-acute environments, ensuring accurate documentation that reflects each resident’s condition, needs, and goals of care. Working closely with an interdisciplinary team that may include physicians, advanced practice providers, therapists, social workers, dietitians, and nursing staff, you will gather and validate clinical data, review medical records, and translate your findings into precise MDS entries that drive care planning, reimbursement, and quality metrics. You will review assessment schedules, monitor key timelines, and help ensure that all MDS submissions are completed within regulatory timeframes and in accordance with federal and state guidelines, payer requirements, and Sanford Health policies. As an MDS RN, you will collaborate with care teams to develop and update individualized care plans based on MDS outcomes, risk indicators, and resident preferences, supporting our mission of compassionate, resident-centered care. You will participate in quality improvement initiatives, audits, and performance reviews related to MDS, case-mix, and quality measures, offering insight to improve outcomes, safety, and the resident experience. You will serve as a resource and mentor to nursing staff on documentation standards, MDS terminology, and regulatory changes, providing education and feedback that support accurate charting and consistent clinical practice.
